Community Support Program

 Projects for Assistance in Transition from Homelessness (PATH)


The emphasis of this program is to engage into the system, obtain housing and benefits for persons who are homeless or at a high risk of homelessness, and are severely mentally ill. The PATH program also monitors the person’s assessed needs and follow-through with services to which they are referred.

 

CSPT (Community Support Psychiatric Treatment) and PATH staff link persons to the appropriate crisis intervention service, which includes services such as CAPS the Mental Health Board’s crises intervention staff- (513) 867-5866.
