Community Psychiatric Support Treatment Worker
(Case Manager)
Transitional Living is recruiting for full-time Case Managers. High School diploma or GED required; Bachelor’s degree and LSW with experience in case management preferred.
Duties include, but not limited to, setting up services for clients (i.e. Medicaid, Social Security, housing, etc.), teaching daily living skills, and other CPST duties as required.
This position is paid on an incentive bonus plan. Ideal candidate will have excellent communication, documentation, time management and computer skills; in addition to coordination and collaboration skills. A valid driver’s license matching the state in which he/she resides, vehicle and insurable driving record (6 points or less) with liability limits of $100,000/$300,000/$50,000 is required.
